Payment Policies:

Everhart Pilates and Movement accept cash, check or credit/debit card payment at the time of service. Checks returned by the bank will be subject to an additional $30 fee.

 

1st Time Appointments:

All clients are required to fill out intake forms, liability waivers/disclosures, and read policies before their first session.

 

Cancellation Policy:

Everhart Pilates and Movement has a 24 hour cancellation policy. We understand that time is valuable to both you and me. Missed appointments or appointments cancelled with less than a 24 hour notice affect both our community of clients as well as your practitioner. Adequate time to fill a vacant spot helps your fellow clients, so please call in your cancellation as early as possible.

 

To cancel an appointment, please call or text 425.501.9061. Please do NOT email your cancellation request.

If you are unable to keep your appointment and do not cancel within 24 hours, you will be charged according to the following guidelines:

• With less than 24 hours notice and No Shows for your appointment you will be charged the full price of the session.

• If you arrive late to your appointment you will be charged the full session price and the appointment will end at the scheduled time.

• If you are using a gift certificate and give less than 24 hours notice or do not show up for your session, the gift certificate will be forfeit.

Appointment Reminders:

You are responsible for remembering your scheduled appointment. However, I attempt to text appointment reminders each weekend for the following week of appointments as a courtesy. It is your responsibility to keep your contact information current with your practitioner. If you do not want to receive reminders please mark N on your intake form.

In the event that you do not receive a reminder notification, the responsibility of attending and paying for your appointment is still up to you. If you miss an appointment that you scheduled, you will be expected to pay for the missed appointment.

 

Information Privacy and Disclosure Practices:

All personal information is held confidential by Everhart Pilates and Movement. Records are secured in a locked facility when not in use by the practitioner. The practitioner will maintain records and no records or information will be released without the written authorization of the client, unless compelled by law (such as subpoenas).

Client files are retained for five years before being destroyed. During this period the client retains the right to request and pay for copies of the file.

 

Client Rights

Clients may request in writing to see or obtain a copy of their records. Clients may request that corrections be made if they identify errors or mistakes. Access to records will be made during regular business hours within 7 days of receipt of written request. A fee may be charged for copying and sending requested records.
